Transition
Preparation for transition out of college begins early in the student’s programme. Post-college provision and the roles played by social services and Connexions or Careers Wales are considered at the first review meeting. The transition process is highlighted to allow the student and parents to commence exploring opportunities for post-college life and begin to make the necessary contacts.
Throughout the learner’s programme the Personal Tutor monitors transition progress through the Transition Checklist, which is kept as a record in the learner’s Personal Tutorial file. This checklist records events on the transition calendar.
Preparation for transition involves a range of topics related to post-college life, independence and active participation in the wider community. The Citizenship and Transition module of the personal development programme aims to prepare every student for transition to the next stage of their lives, whether in further education and training or entry into the world of work. The preparation of a CV is an important component and students are supported to compile their own Records of Achievement.
Work-experience is an important part of the transition programme. All learners will undertake at least one work based training or external work experience placement, appropriate to their needs and abilities. Each workplace away from College has a formal risk assessment.
The transition review meeting takes place during the first or second term of the final year. The student, parents and personal tutor attend this important meeting. Representatives from the home area social services, Connexions or Careers Wales and any other support professionals are also invited to discuss and establish the transition plan. If the Connexions or Careers Wales adviser from the home area is unable to attend, then Shropshire Connexions will make every effort to attend to offer specialist advice regarding further training or employment schemes and other opportunities.
